Most uses of psychotherapy notes, marketing, the sale of health information, and uses not described in this Notice require your written authorization, which you may revoke in writing. If another law provides greater privacy protection, we will follow that law.
If we maintain substance-use-disorder records protected by 42 CFR Part 2, those records will not be used or disclosed in legal proceedings against you without the consent or legal process required by law.
Your Rights
You may request restrictions on certain uses or disclosures of your health information, confidential communications, access to or copies of your records, corrections to your records, an accounting of certain disclosures, and a paper copy of this Notice.We are generally not required to agree to a requested restriction, except when required by law, including certain requests involving services you paid for in full and do not want disclosed to your health plan.To exercise any of these rights, contact the Spine Worx Privacy Officer. Some requests may need to be made in writing.
Our Responsibilities
Spine Worx is required by law to protect the privacy of your health information, provide this Notice of our legal duties and privacy practices, follow the Notice currently in effect, and notify you as required following a breach. We may change this Notice and apply the changes to all information we maintain; the current Notice will be available at our office and on our website.
